With regret, as of October 15, 2005 , we will no longer be participating with MAMSI Optimum Choice, MAMSI M.D.I.P.A., or United HMO.
- If you have an HMO plan with either of these insurance companies, we ask that you choose another primary care provider and contact your insurance company as soon as possible. Recommendations are available at your request.
- If you have a PPO plan with either of these insurance companies, we encourage you to inquire about out-of- network benefits.
